Police are investigating a carjacking on Melbourne's Monash Freeway.(ABC News: Kristofor Lawson)

A woman who stopped to help five teenagers after they crashed on a Melbourne freeway has been set upon and had her car stolen.

Detectives are appealing for public assistance following the frightening encounter, which began when a stolen Toyota Prius was seen driving erratically outbound on the Monash Freeway on April 26.

The car crashed into barriers just before Glenferrie Road about 6:15am, which is when a 33-year-old woman driving behind stopped to help at the crash scene.

Police believe two of five teenagers in the car set upon the woman, forcibly removing her car keys from her hands and then fleeing in her blue Toyota sedan, with the registration 1LK6LL.

The woman, who received minor injuries, was treated by paramedics at the scene.

Police said the Toyota Prius was stolen during an aggravated carjacking in Point Cook on April 24.

"It is believed a group of four male offenders approached a vehicle parked on Adelphi Boulevard while the driver was entering a fast food restaurant, about 9:50pm," Victoria Police said.

"A female passenger in the vehicle was threatened with an edged weapon, before she was able to flee."

That woman was not injured.

Anyone with information or footage of the Monash Freeway incident is urged to phone Crime Stoppers on 1800 333 000 or submit a confidential report via www.crimestoppersvic.com.au.
